Insertion of Fresh Elephant Trunk in Aortic Dissection

by Jeng Wei, MD, MSD, Taipei This video will show you how do we use guide wire to implant a fresh elephant trunk in the descending thoracic aorta. Antegrade insertion of a soft fresh elephant trunk from aortic arch is technically difficult, but it can be easily done if it is pulled with a guide wire from the lower body. The reason why we use fresh instead of frozen elephant trunk is because the incidence of catastrophic stent graft induced new entry (SINE) is still very high. Our experience of using fresh elephant trunk is exciting: (1) No risk of SINE, and (2) Most of the patients had remodeling of the true lumen, i.e. enlargement, soon after the operations. This film will show you how step by step techniques of insertion of elephant trunk by guide wire, and how we remove the guide wire. The vascular ring connector (Vasoring) is used for quick sutureless anastomosis. Since there is grooves on the Vasoring, the anastomosis is much safer than the old intraluminal grafts.
