In partnership with more than sixty health and social care organisations across North Derbyshire, Chesterfield Royal Hospital is aiming to provide patients with a ‘perfect week’ – as part of an exciting project that aims to make sure patients get home from hospital, with the support and care they need, as soon as they are medically fit to leave. At the Royal, the week’s challenges (10th to 18th September) for medical, nursing, therapy and support staff, will involve reviewing patients before 11am, ward teams collecting patients from the Emergency Management Unit, planning discharge dates in advance for every patient, rapid access to diagnostic tests, discharging patients before lunch - and reviewing all patients who’ve been in hospital 14 days and over. It sounds simple – but in practice it requires co-operation from a host of different teams, all working to the same set of standards and making sure everything a patient needs is ready in advance. This could be anything from support aids to use at home, to medication or district nurse appointments. Based on the national ‘perfect week’ concept, North Derbyshire’s trial is thought to be the biggest to date with sign-up from hospitals, commissioning groups, adult social care services, general practitioners, the ambulance service, 111 and GP out of hours; community health services, residential homes and more. The organisations are combining forces to see exactly what could be achieved if they worked in different way. Dr Richard Genever is leading the Peak Flow 1 project and says: “Many of the people we look after are in the later stages of their lives, so time is a precious commodity and we believe that any day spent in hospital without benefit is a day too many. We wouldn’t choose to spend time in hospital unless it was absolutely crucial and for medical reasons, so why would our patients? This is a fantastic opportunity to see what we can do differently to improve patient experience." Results from the seven day ‘experiment’ will be used to progress development plans – and it’s likely there will be several trial to determine what permanent changes the health and social care community needs to make.
