Front line physicians will often times not feel comfortable doing biopsies or removals of lesions on the neck but these are very possible if you respect the surrounding anatomy, progress with appropriate caution and essentially remain superficial while ensuring proper margins. I generally find total removal simpler than an actual biopsy as you aren’t cutting into a cancer or trying to suture through it after the fact. In Canada, at least where I practice, you cannot get plastics or oncology to see these lesions without a tissue diagnosis which forces you to do one or the other. If you are comfortable doing a removal you can do one procedure, achieve clear margins (as we did in this case) and then you are entirely done.
