FMEA is proactive systematic approach to understanding potential failures in products and/or processes and taking corrective action before the failure occurs. It can be used to minimize defects during the introduction of new product designs and/or services or to reduce common failures during the manufacturing or even a business process. FMEA begins with a review of the process and/or product. The team will then brainstorm potential failure modes. Based on process knowledge and historical data, what could fail in the process or product? For example, perhaps we are examining the process for baking a cake. One failure mode could be that the cake sticks to the pan. Next, for each failure mode, list potential effects of each failure mode. In other words, if a failure occurs, what problems could the failure cause? If the cake sticks to the pan when it is done, one possible effect might be that it must be scrapped. Next, write down potential causes. For example, one cause could be that we forgot to butter the pan. After this, we assign ratings for severity, occurrence, and detection for each effect on a scale of 1-10. Severity is a measure of how serious the effect is. For example, if we have to scrap the cake and start over, we are losing time and money, but it is not as serious as an effect like food poisoning. Occurrence is a measure of how likely this is to occur. Usually, based on some historical data, we can determine if this is a common or rare event. Finally, we look at detection. If this is to happen, how likely are we to detect it. For all three categories, we usually use a matrix that guides us in the proper rating. After the ratings are complete, we rank by RPN – Risk Priority Number- from greatest to least. RPN is simply the product of severerity, occurrence, and detection. In other words, we multiply them. After we have these rankings, we begin working on action plans to reduce the highest RPN items. For example, we might begin using non-stick pans to prevent the cake from sticking. We can then recalculate RPN after these actions are taken.
