Focus On Your Health - Community Health Improvement Plan

Diana Lalitsasivimol, PhD, and Anthony Santarelli, PhD, explore how the Live Well Mohave initiative assesses the health priorities of our communities through the Community Health Assessment (CHA) and how CHA data will be used in the Community Health Improvement Plan (CHIP) to improve the health and well-being of the Mohave County community over a three-year period. Background Info: Both the CHA and CHIP are part of an ongoing community initiative called “Live Well Mohave” to improve health and quality of life in Mohave County communities. Since 2012, the Mohave County Department of Public Health and Kingman Regional Medical Center have partnered to lead the Live Well Mohave initiative. Previous community health assessments have resulted in focused initiatives to address health issues such as mental health, substance use, and healthy living.
