In a recent YOUTUBE short, Dr. Anil Rajani asks his audience "What to do about facial fat loss?" His recommendation: Platelet Rich Fibrin injection. His short is big on hype but very short on detail or evidence. It turns out there is a good reason for his ignoring the clinical evidence. Because there is no clinical evidence to support PRF as a standalone treatment for facial fat loss. Zero evidence. So why does Dr. Rajani promote it? It could be because he promotes a combination of Sculptra and PRF injection under the trademark PlasmaSculpt®. Yes, he's trademarked it. He goes around the country "training" physicians on this modality, no doubt sponsored by the makers of Sculptra (Galderma). Dr. Rajani has, however, never published any results in a peer-reviewed journal. In fact, Dr. Rajani does not seem to be a lead or senior author on any peer-reviewed publications listed in PUBMED. He also does not seem to be a sponsor or lead investigator on any clinical trial listed in clinicaltrials.gov. This is not an aberration. It seems as if no one has actually demonstrated that PRF is effective for restoring facial fat. That's right. There is zero clinical evidence. We used a 2022 review (Autologous platelet concentrates for facial rejuvenation, authors, Buzalaf and Levy, Journal of Applied Oral Science) to identify clinical studies of PRF for consideration. Their review listed 6 studies, 4 with PRF as a standalone treatment and 2 with PRF combined with fat transfer. The 4 standalone studies are all weak and none of them shown any effect of PRF on volume restoration. The 2 studies combining PRF with fat transfer are also weak. Neither identifies a primary endpoint for evaluation. And neither study looks at the standalone effect of PRF on volume restoration. So why does Dr. Rajani keep telling us that it works? Most likely he counts on the fact that no one will check and will simply defer to a plastic surgeon. We've offered to debate Dr. Rajani about this and other unsupported recommendations that he makes. So far Dr. Rajani has avoided the merits of the scientific debate and resorted to questioning our qualifications. Hiding behind the moniker of Doctor will get you far but only so far.
