CataractCoach 1173: phaco aspiration of lens milk intumescent white cataract

Intumescent white cataracts can pose a problem with capsulorhexis creation. This is because the cortex has liquefied and pressurized the capsular bag. This means a risk of the dreaded Argentinian Flag Sign which we have covered in detail previously. We need to decompress the capsular bag and this can be done with a manual needle aspiration technique, puncture with the phaco probe, or automated needle aspiration. This video showcases that last technique where a small needle is placed on the aspiration line of a bimanual IA set. This allows immediate aspiration of the lens milk as soon as the anterior capsule is punctured with the needle. The technique works well and all you will need is a coupler to attach the needle to the aspiration line.
