Neck Lift 101: Sculpting Deep Neck Structures | Part 4

https://DrBalikian.com So today, in our ongoing series on neck lifting terminology, we’re going to take a deeper dive into one of the most important yet often misunderstood areas: the deep neck structures. These are the layers beneath the surface, beneath the skin and even beneath the platysma muscle, that play a huge role in the shape and contour of your neck. The deep neck structures are everything that contributes to that central bulging or fullness that sits behind the platysma, which is the broad, hammock-like muscle stretching from the lower jaw down over the neck. In many of the most successful neckline lifts, understanding and addressing these deeper structures is what truly makes the difference between a good result and a great one. Now, when we’re in surgery and we carefully elevate the platysma muscle, lifting that muscular “hammock” to see what lies beneath, we start to uncover several key anatomical features. The first thing we usually see right in the center is subplatysmal fat. This is the fat located below the platysma layer, not the superficial fat you can pinch. This subplatysmal fat is central, and although it can appear bulky, it’s not something we typically remove aggressively. We’ll talk more about why in just a moment. Next, we often find the anterior bellies of the digastric muscles. These are paired muscles that run beneath the chin and can become more prominent or bulky with age, contributing to a thicker-looking neck. We also encounter the submandibular glands, these are salivary glands tucked under the jawline and other connective tissues in the same area that may be full, sagging, or enlarged. When we talk about deep neck lift work—or more specifically, advanced neckline lifts, we’re referring to the sculpting and refinement of all these internal structures. And we don’t approach this like a one-size-fits-all procedure. This is where surgical precision meets true artistry. We customize every step to the patient’s unique anatomy to restore a youthful, elegant neck contour. Every move is deliberate, and it’s all done in a very thoughtful, individualized, and artistic way. So how do we actually do that? Let’s start with the subplatysmal fat. Many people assume we would simply remove all of it to eliminate that central neck bulge, but that’s not the best approach. In fact, removing too much fat from this area can lead to what we call a “cobra neck deformity.” It sounds dramatic and it is. That’s when the central portion of the neck becomes overly hollowed, while the sides look winged or too prominent, creating an unnatural shape. So instead of taking too much, we selectively preserve volume to maintain a natural, smooth contour. Now, let’s revisit the digastric muscles. These muscles help with essential functions like swallowing and jaw movement, but they also contribute to the structural support of the neck. With age, they often thicken, which can result in bulging beneath the chin. When this happens, we carefully evaluate how much of that bulge is due to muscle, and we strategically trim the muscle, not remove it entirely, to achieve a cleaner contour while preserving strength and function. As for the submandibular glands, these are often overlooked but are major players in neck fullness. Over time, they can droop or become enlarged, and because of their location under the jawline, they can affect the overall definition and appearance of the neckline. When appropriate, we reduce them surgically during the deep portion of the lift. This can make a dramatic difference and help us create the smooth, U-shaped contour that defines successful neckline lifts. And here's a critical point: none of these deep neck structures can be addressed with liposuction alone. You can’t suction out subplatysmal fat effectively. You definitely can’t liposuction muscles or glands. These require skilled, hands-on sculpting. That means going in carefully, reducing what needs to be reduced, and relieving some of the “weight” from the platysma so it can lift more naturally.
