Recorded on 8/1/2020 91-year-old female with multifocal adenocarcinoma of the lung Summary: Workup for elderly patients with multi-focal lung nodules suspicious for malignancy should include CT of chest with and without IV contrast to evaluate parenchyma and mediastinum, respectively, as well as PET/CT and PFTs. In elderly patients with suspected multifocal early-stage NSCLC who can undergo surgery, VATS based resection with mediastinal nodal station evaluation remains the standard of care in terms of therapeutic options. If only one of several lesions are resected, the remaining lesions should be surveilled with CT chest without contrast. The frequency of these scans is based on the suspicion of malignancy. If the surveilled lesions in an elderly patient began to demonstrate radiographic evidence of progression, it is reasonable to offer SBRT without additional biopsy confirmation. If the initially resected dominant lesion was found to harbor an EGFR mutation, it is reasonable to surveil the remaining lesions without offering empiric Osimertinib, as there can be between a 50-60% rate of discrepancy in mutational burden between nodules.
