Somerset Urgent Community Response is here 365 days a year to help any patient (over 18), who is at risk of a hospital admission if they are not seen urgently. We provide a two-hour urgent assessment at home (including care homes) for patients who need urgent but not emergency care. This may be due to a clinical condition such as #falls, a new or acute problem such as an #infection or an exacerbation of a chronic condition Our team of healthcare professionals including nurses, advance practitioners and pharmacy technicians can not only provide urgent care but can look into why the patient has had a health crisis so we can try and prevent it from happening again.
