The two long-term friends (more than 35 years) and very experienced implants discuss 3 topics. The 1st topic is most important: Implants with a «Hybrid Surface Concept». The Tissue Level (TL) implants by Straumann, were developed in late 1985 and first utilized in spring 1986 at the University of Bern in Switzerland. The longest documented TL implants are now 39 years! These TL implants showed a low prevalence of peri-implantitis due to the smooth machined implant surface in the transcrestal, peri-implant sulcus area (Buser et al. 2012, Derks et al. 2016). Non-hybrid implants with a micro-rough surface all the way up to the shoulder, however, have a much higher risk for peri-implantitis as prognosed by Dennis Tarnow early on (1993) and as documented by several long-term studies (Derks et al. 2016a,b / Windael et al. 2021). Today, we have a prevalence of peri-implantitis in the market of 20-30%, which is too high and a big threat to implant dentistry. We know several risk factor, but the most important one is an exposed micro-rough implant surface to the peri-implant sulcus. We discuss that we must switch the market to hybrid implants to reduce this risk. It will be a 2nd paradigm shift in the field of implant surfaces. We must do everything to avoid the exposure of a micro-rough implant surface to the peri-implant sulcus. By the way, both surgeons are using today the new TLX and TLC implants, both 4th generation Straumann implants with a hybrid surface concept! They do very well in daily routine, as expected!
