The answer is of course it depends on who you are and who's doing the surgery and so on. But I have some strong opinions so I'm going to give you my strong opinions. I think that breast implants should be put partially but not completely under the muscle. The muscle, your pectoralis muscle ends at your fifth rib. All women, even women with A-cups have their nipples below that area so if it's completely sub muscular, the implants are going to ride too high. Good plastic surgeons will take the muscle off the fifth rib and lower the inframammary fold below that position, depending on where they want the fold to fall. So breast implants should never be placed in front of the muscle because when you do that, you have an increased chance of hardness, they look really inappropriate because there's a shelf on top of them, it is the old way to do things and not really appropriate in today's world.
