Exertional compartment syndrome requires fasciotomy to release the fascia so the muscle has room in the compartment to swell. Blind release of the fascia has the associated risk of injury to artery and nerves. Open release requires much larger incisions. The Segway system has a guide created for cubital tunnel release that allows for its use in fasciotomy in a minimally invasive technique. I make 2 incisions laterally and a single medially. The device allows me to dissect the soft tissue off the fascia with the dilators and then insert the guide above and below the fascia. The guide allows you to advance on the fascia while cutting it and clearly visualize and potential neurovascular structures the field. The retraction of the skin and muscle by the guide affords me the greatest margin of safety and the most bloodless field.
