Using the Snellen chart, nurses will position their patients 20 feet away from the chart and instruct them through a variety of tests where both the right and left eyes are assessed separately and then both together to determine the patient's visual acuity. There are many reasons why a patient will need a visual acuity exam completed. There may be complaints of irritation or signs of infection, injury, or declining function. This is a baseline exam that may result in a referral to an optometrist or ophthalmologist for further assessment. Understanding how the Snellen chart is graded will help nurses accurately educate and document their findings.
