Among abdominal trauma, the liver is the second most common organ that gets injured. This surgery lecture also discusses the surgical approach to liver trauma. ▬ 🔬 Liver Trauma ▬▬▬▬▬▬▬▬▬▬ The liver is the largest organ in the human body and one of the largest glands. Liver damage can lead to severe circumstances and is categorized into blunt injuries and penetrating injuries. Liver laceration symptoms requiring diagnosis include lower chest and upper abdominal stab wounds, severe crushing injuries to the lower chest or upper abdomen, combined rib fractures, and haemothorax. Clinical radiology techniques such as FAST and contrast-enhanced CT scans are crucial for diagnosing liver trauma/laceration. Management of penetrating injuries begins with airway management, along with maintenance of breathing and circulation. Various management approaches follow, including thromboelastography to assess blood coagulation. The lecture illustrates different levels of coagulation: normal, delayed clot formation, reduced clot strength, clot lysis, and hypercoagulable states. For blunt injuries, liver trauma management involves initial resuscitation and the management of haemodynamically unstable and stable patients. The surgical approach to liver trauma is also discussed in detail. In this General Surgery lecture, the liver trauma management algorithm is explained step by step, covering both penetrating and blunt injuries.
