Procedure : Foreign Body Removal (Bronchoscopy) A one year's six months baby girl presented in SQUARE HOSPITAL LTD Dhaka Bangladesh with history of FB Aspiration (Nut)? Medication : GA ROUTE: Trans LMA FINDINGS: VOCAL CORD: Normal in motion and phonation. TRACHEA: Normal. CARINA: Normal and sharp. RIGHT LUNG: Foreign body (one part of a piece of nut) measuring 0.8cm long was seen over just opening of the right principal bronchus. Profuse thick mucus secretion came out from above and behind the foreign body. LEFT LUNG: Normal. BAL: Taken. IMPRESSION: Foreign body (one part of a piece of nut) in right principal bronchus. THERAPEUTIC INTERVENTION: Foreign body was removed by dormia basket with flexible bronchoscopy. The common foreign bodies are vegetable foreign bodies like pieces of peanuts, betelnut (supari), almond (badam), kaju, etc. Others can be small parts of toys, metallic objects (screws, nuts, etc). The common symptoms are bouts of cough after eating a food item (peanut, etc), choking (breath hold, turning blue), unexplained difficulty in breathing, wheezing (a whistling sound from the chest). Doctor may find a difference in the breath sounds of the lungs and a wheezing sound on hearing with a stethoscope. Some non-food items (metallic objects like pins, screws, other small metal pieces, etc. ) can be seen in the airway in a Xray Chest. However, most food, vegetable matter and plastic toys cannot be seen on a Xray. Other changes in the Xray might suggest a foreign body like collapse of a part of the lung. It should be remembered that many children with foreign body aspirations can have normal chest xrays. The only method to confirm or refute the presence of a foreign body in the airway is to perform a flexible bronchoscopy. Large foreign bodies can be life threatening as they can block the voice box (larynx) or the windpipe (trachea). Small foreign bodies can lodge in the windpipe or the other smaller airways (bronchi). This can lead to obstruction of a portion of the lung, infections and later can cause serious long term damage to the lungs.
