Phacoemulsification in topical anesthesia High myopia IOL power +10.5 D AC depth almost 4mm ( deep anterior chamber) Very soft cataract - Difficulties 1. AC is very deep so you have to do everything at little deeper plane. It looks like you are operating in a well. 2. Classical chopping is not possible sometimes due to very soft cataract. 3. Eye is very soft in high myopes. 4. Rhexis must be large enough. Pearls for successful outcomes 1. Make a rhexis of atleast 5.5mm 2. Use sodium hyluronate in rhexis to get good control over capsule which is very elastic 3. Hydrodelineation helps to separate inner most nucleus which can be now easily emulsified without classical chop. 4. Rest all other steps are same as any standard phaco procedure. 🙏🙏🙏 Dr. Sanjay Gupta
