Explantation of dislocated IOL with fibrotic capsules and Yag capsulotomy and new IOL implantation
This is a case of traumatic cataract, having surgery more than twenty years ago, having implanted a PMMA IOL, followed later on by IOL dilocation due to fibrotic changes of the capsular bag and Yag Capsulotomy, with multiple Yag punctures of the IOL. The patient was unhappy because of the cosmetic appearance: the pupil was descentrated and with inesthetic "white large spots". We explanted the PMMA IOL, performed anterior vitrectomy, cleaned all the white fibrotic tissue and cortex remnants, keeping a capsular peripheral ring sufficient to safely receive a 3 pieces IOL (6mm/13mm).
