Hand surgery has been traditionally an important part of training in plastic surgery. In fact, my former mentor and residency program director - world famous plastic surgeon Ivo Pitanguy (1926-2016) set up the 1st ever hand surgery service in Latin America in the 1950s. Here, I examine a gentleman who complained of pain and swelling at the base of the thumb. This pain was triggered by exercises that involved extensive wrist motion (in this case , he was a jujitsu player and a boxer/mixed martial artist). There was also a sensation of catching or popping when moving the thumb, especially when he lifted his newborn infant at home. I perform the Finklestein's test which was postive. A positive test result occurs when the patient experiences increasing pain at the radial styloid tip and is pathognomonic of De Quervain’s tenosynovitis. De Quervain’s tenosynovitis is a painful condition that affects the tendons in your wrist in the 1st extensor compartment. It occurs when the 2 tendons around the base of your thumb become swollen. The swelling causes the sheaths (casings) covering the tendons to become inflamed. This puts pressure on nearby nerves, causing pain and numbness. In the video, I also demonstrate the corticosteroid injection technique for De Quervain’s Tenosynovitis. It’s essential that you treat de Quervain’s. If left untreated, it can cause permanent damage to the movement of your wrist and thumb or cause the tendon sheath to burst.
