The type of implant design affects the shape of the associated polyethylene tibial insert. The shape of the polyethylene tibial insert varies greatly due to the myriad TKA designs. One of the challenges during MRI interpretation is determining whether the appearance of the polyethylene tibial insert indicates an abnormality, such as polyethylene wear, or reflects the manufacturer’s design. ✅ Posterior-Stabilized (PS) Knee Prosthesis The posterior-stabilized (PS) knee prosthesis is of an unconstrained design tailored for patients in which preservation of the native posterior cruciate ligament is not possible. This design features the post-cam mechanism to substitute for the function of the posterior cruciate ligament. In these designs, the femoral component has a cam that engages the vertical post on the polyethylene tibial insert to promote rollback of the femur on the tibia and improve flexion. ✅ Posterior Cruciate-Retaining Designs Posterior cruciate-retaining designs rely on the native PCL for stability during flexion, and the polyethylene tibial insert does not feature a post.
