Difficult Extraction in an older Patient: Buccal Trough + Sectioning of Wisdom Tooth

This is a difficult one. Older patients with teeth like this (think about booking more time for the case). In real time this took me about 20 minutes (which is a while for me) so plan for more time if you think you need it. 📌Remove the overlying tissue so you can see the tooth well. 📌Buccal trough with my 703 fissure bur at 80,000 rpm 1:2. Using a Bien Air Chiro Pro L Straight handpiece. The tooth doesn’t elevate after.. 📌Buccolingual sectioning. I start more mesial as the tendency is to drift distally. This way I hit the furcation better. This works well for me but find what works best for you. The tooth still does not elevate out.. 📌Using various elevators to get the roots moving but nothing is really working. My offset spade, 77R elevator.. 📌I get more movement on the mesial root so I use a small rongeurs on it. Taking my time it starts to move out. I usually get the distal root out first but this helps give me room for the distal root. It also is a psychological win which is nice. It had two roots.. 📌I tried again on the distal root with elevators and even got out the Cryer’s. I felt more movement so I went back to the small rongeurs. They kept slipping and weren’t getting the root moving so I turned them and grabbed the root mesiodistally and that worked.
