Plastic surgeon Adrian Richards shows how plastic surgeons assess breast shape and droop. In this video Adrian Richards draws diagrams to show the different types of breast shape and droop and what plastic surgeons aim to achieve when performing breast surgery. First of all, I'm going to illustrate this is the natural breast shape, these are the clavicals and the nipples are in these positions. So in a normal situation the distance from here if you put your finger here, you'll feel the notch between your clavicals there, the distance from the sternal notch to the center of the nipple in an average sized female would be 21 centimeters, that should be 21, that should be 21 and that should be 21. So for a 5ft5 female, studies have shown that that distance should be a triangle with all the sides the same being 21 centimetres. Also if you look at your arm that distance should be half the distance from the shoulder to the elbow so the nipple should lie 50% below and 50% above so that is the ideal position of the nipples. In that case, if we have a relatively small bust with the nipples in the normal position this is from the side, we notice that distance is 21 centimeters although it will be slightly longer for a taller person. In this position we have no breast droop so what we can have with women after they have had babies is they can lose volume in this area here but in other cases we can lose volume and we can have a droop of the breast which is a lowering of the nipple position, in this case the nipple lies above the fold, this is the breast fold which is called the infra mammary crease IMC, the crease will be here behind the breast. This is no droop the medical word for droop is ptosis. The ideal position is the nipple lying above the fold. The next position is nipple lying at the fold and finally nipple lying below the fold. If the nipple lies above the fold you don't need an uplift, if the nipple lies at the fold you can consider an uplift, if the nipple lies below the fold if we do do an breast implant the implant will tend to go to the lowest part so you will get a fullness in this area but we wont get a very nice breast shape. So in essence if you are this condition you don't need an uplift, we can restore the volume with an implant alone, this case we consider either breast implant for a very natural appearance but we may need to perform a small uplift to elevate nipples with minimal scarring but in this case severe ptosis we would need to restore the volume and elevate the nipple. So I hope that's clarified some of the issues about breast ptosis and about how a surgeon would look at the appearance of your bust. zuffbloss
