A 42-year-old, Female presented with complaints of Jaundice, Abdominal pain, and Weight loss. Her USG was s/o dilated CBD, and distal CBD stricture. She was referred to us for EUS evaluation which was s/o approximately 20*19 mm hypoechoic mass lesion in the head of the pancreas without any obvious vascular involvement. We carried out EUS FNA and FNB with a 22 G needle for cytology and HPE. In view of the potentially resectable lesion and young age, a 10 Fr biliary stent was placed for biliary decompression prior to definite surgery. Take home message- EUS FNA FNB and ERCP in the same setting is feasible, cost-effective and less time-consuming. EUS FNA FNB result comes the same day with the availability of Rapid On-Site Evaluation (ROSE).
