Are you considering a facelift or neck lift and wondering what the SMAS layer is—and why it matters? In today’s video, we’ll break down the Superficial Musculoaponeurotic System, or SMAS, this is a crucial layer of tissue beneath the skin that connects facial muscles to the skin. It plays a key role in maintaining facial structure and is central to most modern facelift and neck lift techniques. The SMAS layer itself is comprised of muscles and fibrous tissue that stretches from the platysma in your neck to the galea aponeurotica of the scalp. It connects your facial muscles to the skin and is crucial for lifting sagging areas like the jowls, cheeks, and neck. Facelift procedures that target the SMAS lift and tighten this deep layer along with the skin. This creates more natural, long-lasting results compared to skin-only lifts. There are two parts to the SMAS, the mobile and non-mobile SMAS: Mobile SMAS - This part is more prone to aging due to its looser attachment to deeper structures. Non-mobile SMAS - This part is more fixed and resistant to aging effects. Now let’s look at three common facelift techniques that manipulate the SMAS: 🔹 SMAS Plication Facelift This method folds and stitches the SMAS onto itself. It's less invasive with a faster recovery time but requires more skin undermining, which can increase the risk of complications like seromas and doesn't provide as long-lasting results as deeper techniques. 🔹 High Lateral SMAS Facelift This technique lifts the non-mobile SMAS vertically, enhancing the midface and jawlinewithout releasing deeper ligaments. It’s less aggressive than a deep plane lift and often has quicker recovery, but still involves skin elevation and carries a risk of skin necrosis in high-risk patients. 🔹 Extended Deep Plane Facelift This is the gold standard. It releases the retaining ligaments of the mobile SMAS and lifts it along with the skin as one unit. This comprehensive approach delivers the most natural and durable results, especially for patients with advanced aging.
