Is it realistic to hope for successful IVF after fibroid and endometriosis removal? Fibroids are benign, or noncancerous, growths that form on or in the uterus and can grow in both size and number. Some fibroids are smaller than a peppercorn, while some have grown to more than 20 pounds. By age 50, roughly 80% of women will have been affected by them. People with uterine fibroids may be asymptomatic, or they may suffer from pelvic pain, heavy and extended menstrual bleeding, backaches, frequent urination and/or constipation. With endometriosis, the same kind of cells that line the inside of the uterus grow outside of the womb. This endometrial matter can grow into lesions and implant anywhere in the pelvis, including on reproductive and other organs. The lesions bleed with menstrual cycles. As with fibroids, endometriosis can exist without causing symptoms, but common complications include heavy menstrual bleeding during and between periods, pelvic and lower back pain, pain during sex, gastrointestinal issues and fatigue. How can fibroids and endometriosis affect fertility and IVF? Fibroids, especially submucosal fibroids that grow within the lining of the uterus, can interfere with conception and embryo implantation or result in miscarriage. About half of all endometriosis patients experience infertility. The condition’s chronic inflammation and scarring can block the fallopian tubes and impede fertilization and implantation. In addition, a type of ovarian cyst called an endometrioma can destroy ovarian tissue, reduce the number of stored eggs and harm egg quality. If infertility is suspected, or prior to starting IVF, CIGC specialists recommend being evaluated and treated for either of these conditions to create the best possible chances for pregnancy success. Can fibroids and endometriosis be permanently cured without having a hysterectomy? Hysterectomy, the surgical removal of the uterus, will take away existing fibroids and eliminate future growths, which is why many OBGYNs recommend hysterectomy as a permanent treatment for fibroids. However, without the uterus, fertility is no longer an option. With myomectomy, rather than removing the entire uterus, individual fibroids are cut out one at a time, with the goal being a preserved womb that is capable of future fertility. During a myomectomy, a skilled specialist can remove uterine fibroids and endometrial lesions at the same time. Unlike with hysterectomy, however, fibroids can grow back. It is also possible that more endometrial lesions will develop later—and this is true even after hysterectomy, since lesions don’t only form in the uterus—but the goal is to allow for pregnancy before any regrowth occurs. This is the scenario Jeanette experienced. How did CIGC patient Jeanette achieve IVF success with severe endometriosis and fibroids?
