And we’re gonna talk about the management of Bilateral Vocal Fold Paralysis. So Bilateral Vocal Fold Paralysis is when the vocal folds don’t move and typically they're stuck in the middle. When that happens, patients have problems with breathing. The reason why people choose Cincinnati Children’s is because of two reasons. Our multidisciplinary care of patients with bilateral vocal fold paralysis and our outstanding airway reconstruction history over the last 30 to 40 years. So, what can you expect from your first visit at Cincinnati Children’s. First, you're usually get a phone call from one of our airway nurse practitioners from our division to find out more history about your child. Once we get that information, our nurse practitioner collaborates with one of our surgeons, like me, to figure out what we should do for your initial evaluation. Once we create that initial evaluation we move on afterwards to thinking about surgery. When we think about surgery for Vocal Fold Paralysis, we like to balance…breathing, voicing and swallowing. And we like to obtain a result that balances those 3 things. There are many surgical options for Bilateral Vocal Fold Paralysis. We’re gonna talk about two of them Endoscopic Posterior Grafting and Open Posterior Grafting Grafting is when we take a piece of rib cartilage and insert it in to an incision in the back part of the voice box to make the voice box bigger at the level of the vocal cords. Endoscopic Grafting is when we do a surgery through the mouth without doing incisions on the outside of the neck. Open is when we make an incision on the neck to open the larynx to insert the graft via an open approach. So, why do we choose these twp options at Cincinnati Children’s? Those two options offer us the best advantage for balancing airway, voicing and swallowing. It also offers us the opportunity to improve the voice in the long term by doing an operation to reconstruct the voice when we need to.
