Women confronted with the option of having breast reconstructive surgery face a difficult decision that raises many questions about the procedure itself and the potential outcome. Miami Cancer Institute provides the most advanced surgeries and overall care for cancer patients, including breast reconstruction for women who have undergone mastectomies. "Breast reconstruction has been proven to really improve the quality of patients' lives by removing the disfigurement and deformity associated with mastectomies," says Miguel A. Medina III, M.D., director of microsurgery at Miami Cancer Institute. "It helps patients move on. Patients that have had breast reconstruction have shown to have much lower levels of anxiety and depression after their cancer care. And they just have a much more positive outlook on life." [Transcript] (gentle music) [Q&A with Miguel A. Medina III, M.D. Plastic and Reconstructive Surgeon Director of Microsurgery at Miami Cancer Institute] [Dr. Medina, M.D.] Hello, I'm Dr. Medina and today we're gonna talk about breast reconstruction surgery at the Miami Cancer Institute. [What are some positive aspects of breast reconstruction after a mastectomy?] [Dr. Medina, M.D.] The most positive outcome of breast reconstruction is to make a patient feel whole again after cancer has taken away a part of their body. It's really important for us to be able to restore that sense of femininity after their cancer operation. [How does breast reconstruction affect the quality of life for breast cancer patients?] [Dr. Medina, M.D.] Breast reconstruction has been shown to really improve the overall quality of patients' lives by removing the disfigurement and deformities that are associated with a mastectomy and what that really does is it helps a patient move on. Patients that have had breast reconstruction have been shown to have much lower levels of anxiety and depression after their cancer care and they just have a much more positive outlook on life. [What are some advancements available to patients at Miami Cancer Institute?] [Dr. Medina, M.D.] We're able to offer patients treatment with their own tissues so autologous flaps. We're able to offer implant based reconstruction and we're also able to offer adjuvant reconstructions such as things like fat grafting. One of the things that we have a particular focus here is in microsurgical breast reconstruction and that really involves transplanting tissue from one part of the patient's body to another. When we do these tissue transplants, we do no harm in the donor site and we've created a successful breast reconstruction. [What are the pros and cons of these innovative procedures?] [Dr. Medina, M.D.] These procedures are in general are very effective. The real issue is that not every patient or not every breast reconstruction is the right fit for every patient. We're able to transplant tissue not just from the belly but other places, potentially the buttocks or the thighs. These are really tailored per patient. [What is your advice for women deciding whether or not to have breast reconstructive surgery?] [Dr. Medina, M.D.] There's no requirement to perform breast reconstruction and that this is really intensely personal decision and each patient has to make it on their own. Certainly any woman that doesn't feel like they want a breast reconstruction, it's totally appropriate for them not to have a breast reconstruction. In general, people feel whole after these operations and they feel that they haven't lost a part of their body in the same way that those patients that have not had a reconstruction. [What is the breast cancer patient journey like at Miami Cancer Institute?] [Dr. Medina, M.D.] We have a tremendous luxury here at MCI in that we have everyone under the same roof. We are able to have them see everyone within a very short period of time. The typical patient that has an abnormal mammogram is seen by a breast surgeon here. If they need biopsies or additional imaging, that can all be performed in this facility. After we have a firm diagnosis, those patients then get referred to a medical oncologist and if they have indications for breast reconstruction or for radiation, they are sent to see the radiation oncologist and the plastic surgeons as well. In addition, we have excellent cancer support services and survivorship services and if anything were to recur or come back up in the future, the exact same team is all in place and ready to take care of them.
