Murphy, M.D., Christopher J. Hawryluk, M.B.S., Ayham Jaber, M.D., Tyler J. Uppstrom, M.D., and Peter J. Millett, M.D., M.Sc. Video Summary: Summary: The patient, who injured his left shoulder in a BMX accident, experienced two dislocations within consecutive days. The second dislocation was reduced by an orthopedic surgeon, but he continued to have pain and instability. Imaging revealed a bony Bankart lesion and anterior labral tear. During surgery, the glenoid fracture fragment was elevated and visualized using a 70° scope. Anchors were strategically placed to secure sutures around the bone fragment and associated capsulolabral tissue. A combination of knotless fiber anchors and push lock anchors created a double-row bridging construct to compress and stabilize the fracture. Additional anchors were used to address labral tearing as needed. The final construct successfully restored the articular surface, reduced and stabilized the bone fragment, and repaired the capsulolabral tissue, resulting in excellent glenohumeral stability. Postoperative imaging confirmed proper fixation, and rehabilitation was initiated immediately after surgery.
