An irido-zonulo-hyaloid-vitrectomy (IZHV) is a surgical procedure combining a vitrectomy (removal of vitreous gel) with an irido-zonulo-hyaloidotomy to treat malignant glaucoma, also known as aqueous misdirection. The surgery creates a path between the posterior and anterior segments of the eye, clearing the trapped aqueous fluid and relieving the pressure causing the shallow anterior chamber and high intraocular pressure. It involves using a vitreous cutter to perform a complete vitrectomy, a zonulectomy to cut the zonules (fibers that hold the lens), and a hyaloidectomy (removal of the anterior hyaloid membrane), while also creating peripheral iridotomies to ensure a stable pathway for fluid to drain.
