This patient had a hysterectomy in the past, but her ovaries were left behind. On examination, we found that the ovary is still functional. The fallopian tube is swollen due to a blood-filled collection caused by endometriosis, known as hematosalpinx. More importantly, a large endometriotic nodule was found stuck to her bowel. This was missed in her earlier surgeries and is the main reason for her ongoing severe pain. She has already undergone three surgeries, but none by endometriosis specialists. As a result, key parts of the disease were left behind. Now, she needs a fourth surgery to get relief. This case is a reminder of why it’s so important to consult a specialist for endometriosis. Accurate diagnosis and complete disease mapping are essential to treat it properly and avoid repeat surgeries.
