A hernia in the calf usually refers to a muscle hernia — a relatively rare condition where part of a muscle bulges through a tear or defect in the surrounding fascia (the tough connective tissue that covers muscles). Symptoms Visible or palpable lump in the calf that becomes more noticeable when standing, walking, or contracting the muscle. The lump may disappear or shrink when the leg is at rest. Aching, tightness, or pain — especially during exercise. Occasionally, numbness or tingling if nearby nerves (like the superficial peroneal nerve) are compressed. Diagnosis Physical exam — the bulge may be seen or felt when the muscle is tensed. Ultrasound — dynamic imaging (during movement) helps visualize the herniation. MRI — can confirm the defect and rule out other conditions (e.g., cyst, tumor, varicose vein). Treatment Options Conservative (first-line): Rest, activity modification. Compression sleeves or stockings. Physical therapy for strengthening and flexibility. Avoid repetitive high-impact activities that worsen symptoms. Surgical (if persistent or symptomatic): Fasciotomy (releasing tight fascia) or Fascial repair (closing the defect with sutures or grafts). Surgery is typically reserved for athletes or those with pain or nerve symptoms. Prognosis Most cases are benign and can be managed conservatively. Surgery has a good success rate but is only necessary in more severe or symptomatic cases.
