Multifocal Lens Implants in RK cataract surgery: Pearls and Pitfalls - Gulani Vision

ReStor multifocal lens implant and radial keratotomy cataract patients: In this video I will share some of my previously published concepts of cataract surgery and previous RK patients. The important thing in these cases is to determine the site of entry usually corneal in between the most spaced, adjacent RK incisions. Once the entry is made it is important to maintain the phaco tip in the same position while maneuvering the entire cataract into the phaco tip with a feeding mechanism using the second instrument. My cataract technique of choice in RK cases is the lateral stuff technique I have previously described. Begin with a small capsulorrhexis that can be gently spiraled into an adequately sized rhexis to allow the cataract to be hydrodissected completely out of the bag or tilted with a second instrument as in this case to provide a lateral edge that can be addressed with the phaco tip. Watch this cataract Carousel laterally into the phaco tip and also maintain the phaco tip without much movement thus preventing distortion of the RK incisions. In these RK eyes remember even though they have presented with hyperopia these are anatomically myopic eyes with deep anterior chambers prone to hiding pieces of the nucleus in subincisional areas especially given use of viscoelastic in these cases. Gently move the second instrument to the subincisional area below the phaco tip to sweep and look for any residual pieces of nucleus as in this case. Similar protocol needs to be followed with the irrigation aspiration tip with very minimal movement once engaged in the incision. I also rock the eye medial to lateral just to disengage any piece of nuclear that may be hiding behind the iris in this way performing a second check before completing my case. Another important point to remember is the visual distortion from the RK incisions that you need to overcome with the background light reflex and make sure you're multifocal lens is nicely centered within the capsular bag. Remove all the viscoelastic behind the lens implant and corneal incisions are sealed by hydrating the incision and the adjoining RK incisions to a watertight closure. http://www.gulanivision.com http://gulanivision.com
