In this neuropathology lecture, Professor Zach Murphy presents a detailed breakdown of medullary brainstem lesions, focusing on the clinical localization and vascular anatomy behind medial medullary syndrome and lateral medullary syndrome (Wallenberg syndrome). This lecture bridges neuroanatomy with clinical presentation to help you confidently recognize and differentiate these classic stroke syndromes. We begin by reviewing key medullary structures including the corticospinal tract, medial lemniscus, hypoglossal nucleus, spinothalamic tract, nucleus ambiguus, and vestibular nuclei. Medial medullary syndrome, typically caused by occlusion of the anterior spinal artery, presents with contralateral hemiparesis, contralateral loss of proprioception and vibration, and ipsilateral tongue weakness due to hypoglossal nerve involvement. In contrast, lateral medullary syndrome is most often due to posterior inferior cerebellar artery (PICA) infarction and produces a complex constellation of symptoms including dysphagia, hoarseness, vertigo, nystagmus, ipsilateral facial sensory loss, contralateral body pain and temperature loss, and ipsilateral ataxia.
