Central Pontine Myelinolysis is a neurological disorder that commonly occurs when sodium deficiency or as called is treated too rapidly. When a patient have hyponatremia and suddenly takes in a lot of sodium the patient may develop this condition. What happens in this situation is that the increased sodium levels in the blood increases the tonicity of blood. now if you remember, if a solution is hypertonic than it pulls water from the cells. This same thing happens here, water from the brain cells are pulled out into the blood which leads to the cells becoming dehydrated. Thus the cells are destroyed. Now in nerve cells, the part that actually gets damaged is the myelin sheath and the part of the brain that mostly gets affected by this is the pons part of the brain. Thus the patient may show show symptoms of difficulty in speaking, swallowing and co ordinating alongwith depressed level of awareness, confusion, paralysis of arms and legs and stiffness. Now if this damge to myelin is too severe, than patient can even enter into coma or die. This ois the reason why people recieving 3% Nacl through IV must be carefully monitored for neurological symptoms and the reason why 3% Nacl is considered as High risk medication Extra care should be taken if correction of hyponatremia is done in patients with liver transplant, chronic alcoholic and malnourishment. Patient with Liver transplantation have shown the highest risk of developing this disorder and it is more common in females in comparison to males. That 's about it , in Central Pontine Myelinolysis.
