As we blink, the excessive tears are pumped into the nasolacrimal passage and the excessive tears enter into the nose and to the throat. That’s the reason why when eye drops are applied into the eye, one may get bitter taste into the nose. Occasionally this passage can get blocked, there are various stages where it can get blocked. It can occur in children, called as congenial nasolacrimal duct obstruction. In this the passage is not opened. So a child is bought to the surgeon complaining of excessive tearing from the eye. There is no discharge or redness in the eye. This said that the passage is blocked in the initial stages it can be controlled by doing a massage. A properly done massage atleast 2 to 3 times a day, atleast 5 minutes in a proper manner, the passage usually opens out. If the passage is not open, even after the child passes 1 and half years, probing and syringing has to be done. This is usually done under general anesthesia and the nasal passage is dilated a syringing is done.Usually this opens out. Most of the time if proper massage is done, a child may not require probing and syringing. In adults, the passage can get blocked because of pure hygiene or because of infections. This can be divided into two types, one is acute dacrocystitis , where there is acute pus formation, in the lacrimal passage and this requires immediate treatment in the form of painkillers, antibiotic eye drops, antibiotic oral tablets. This may take a few days, along with warm compresses. This may take few days to resolve, but this can repeating again because once the passage is blocked, one can get repeated infections. In order to prevent repeated infections, one needs to go for permanent treatment called as dacrocystorhinostomy. If the passage gets blocked, one can get repeated watering and infection. This is called as chronic dacryocystitis. Chronic dacryocystitis also requires a surgical intervention which is called as Dacryocystorhinostomy. Here the blocked passage, an artificial new passage is created for the excessive tears into the nose. This is what is called as Dacryocystorhinostomy, which can be done externally from the skin surface, which is known as external Dacryocystorhinostomy or external DCR or through nose called as endoscopic Dacryocystorhinostomy. This can also be done by an eye surgeon or ENT surgeon. Nowadays lasers are being used to create artificial passage. So this is called as laser Dacryocystorhinostomy. So for all the procedure if an artificial passage is created, bypassing the blockage and a new passage is created for the excessive tears to drain from the eye towards the nose, so this is called as Dacryocystorhinostomy procedure. One needs to do regular follow ups, occasionally this passage may be blocked and during initial stages your surgeon may do repeated syringing that is flushing water or flushing fluid to the eye into the massage so it keeps patent, so this is called as Dacryocystorhinostomy. There are various modifications available for Dacryocystorhinostomy, depending upon where the blockages, called as intubation Dacryocystorhinostomy where a silicon tube may be passed. So depending on the passage the surgeon may decide whether a Dacryocystorhinostomy procedure or combined with intubation, whether it can be done externally or though endoscopic, it depends upon the surgeon. So treatment for chronic dacryocystitis is only Dacryocystorhinostomy surgery. If it is not done, a person may repeatedly keep on getting infection. So permanent treatment is required for Dacryocystorhinostomy procedure.
