Angina, also known as angina pectoris, is a type of chest pain that occurs when there is a temporary lack of oxygen-rich blood flow to the heart muscle. It is typically a symptom of an underlying heart condition, most commonly coronary artery disease (CAD), where the arteries that supply blood to the heart become narrowed or blocked by atherosclerosis (plaque buildup). Angina is classified into different types based on its characteristics and triggers, which include: 1. Stable Angina: This is the most common form of angina and occurs predictably during physical exertion, emotional stress, or after a heavy meal. Stable angina follows a consistent pattern and is often relieved by rest or nitroglycerin, a medication that helps dilate blood vessels and improve blood flow to the heart. 2. Unstable Angina: Unstable angina is more concerning than stable angina as it occurs more frequently, is unpredictable, and can happen at rest or with minimal exertion. It is considered a medical emergency because it may signal an impending heart attack (myocardial infarction). Unstable angina requires immediate medical attention to assess and manage the underlying cause of the increased symptoms and to prevent a heart attack. 3. Variant (Prinzmetal's) Angina: Also known as vasospastic or coronary artery spasm angina, this type of angina is caused by temporary spasms or constrictions of the coronary arteries, reducing blood flow to the heart. It often occurs at rest and may not be related to physical exertion or emotional stress. Nitroglycerin and calcium channel blockers are commonly used to prevent and treat variant angina by relaxing and dilating the coronary arteries. 4. Microvascular Angina: Microvascular angina, also known as cardiac syndrome X, affects the smallest blood vessels in the heart (microvasculature) and can cause chest pain during exertion or stress. The coronary arteries themselves may appear normal on angiograms, making diagnosis and treatment more challenging. Medications such as beta-blockers, calcium channel blockers, and antiplatelet agents may be used to manage microvascular angina.
