Auto-Transplantation Case: Pt presented with #31 SIP/SAP. Deep isolated 9mm probing on the ML, after accessing a crack extending down the ML canal of #31 was detected. C&S was done to address the patient’s symptoms and CBCT was taken. We proposed transplanting #32 into #31’s socket to the patient as an option before considering an implant. The similar anatomy of both teeth and measurement from CBCT made this more of a favorable indication. #31 and #32 were extracted using forceps only, #32 was transplanted into #31’s socket with no adjustment to the socket and fit very well. Extraoral Time was under 2 min. Teeth were stabilized with glass fiber splint. Transplanted tooth was accessed after 10 days, C&S and CH was placed. 3 weeks later the tooth was obturated and splint was removed. Pt is asymptomatic and the tooth is stable with no mobility. Patient is placed on recall. Lit review: In 220 autotransplant cases with average 6 yr follow up, the success rate was 95%. The predictive factors for success were: short extraoral time, less PDL damage/Socket manipulation and patients younger than 40 yrs of age (Tsukiboshi 2002). I also included a video showing step by step of Endodontic treatment start to finish of the transplanted tooth. This is another scenario where we can utilize natural dentition to replace hopeless teeth before we consider an implant. Case selection is key of course.   / bfsjut4gvkh
