The main challenge to this is remembering the 5 pillars 3 times. There is a special at every 10% hp; each special will happen 3 times. I personally recommend running towards 1(lowest hp pillar) immediately and then memorizing 4 and 5. The moment they are memorized leave the shadow world with the fragment. The pillar you grab for 2 and 3 should be the furthest pillar from 4 and 5 possible. Get the Blackstone fragment from Ketla's house. Ketla is the dwarf from the dt2 quest.
