we describe herein the procedure commenced as clear cornea phacoemulsification in a high myope with advanced cataract despite relatively young age. Intraoperatively the density of the cataract and compromise of the posterior capsule negated converting to extracapsular removal of the remaining cataract fragments through an extended 6mm incision (modified Blumenthal mini-EECE technique). Successful cataract removal was completed by a limited anterior vitrectomy. Intact anterior capsule allowed a 3 piece IOL placement into the sulcus. The incision was placed so to use the expected cylinder induction as a means to reduce the pre-operative corneal astigmatism. The visual outcome was rewarding as the lessons learned from this useful case
