This happens because of the complex nature of the extensor mechanism as it passes over this middle joint. I’ll just draw it for you. So the tendon comes down as one, and then at that point it splits into three different tendons, two of the tendons passing down the sides of the finger, and the central tendon, known as the central slip, finishing just at the point of the middle knuckle. Now, in a rupture of that tendon as it passes over the middle knuckle, it tends to cause the finger to sit in a bent position and you can’t straighten it back out. So in order to treat that, what we need to do is we need to be able to pull that middle joint out into a straight position. What this does is this helps the little tendons that sits on the sides of the fingers to sit back up on top. That’ll help straighten out the finger, and allow this central tendon the chance to heal. And we make a barrel or a circumferential splint right the way around the injured knuckle. So the splint goes on, sits over the top of that injured knuckle, but it’s really important for your rehab that you try and bend this tip as much as you can.
