In cases of chronic retinal detachment with severe anterior PVR, performing a 360-degree retinectomy is the best approach to ensure an optimal surgical outcome. Circumferential retinectomy is performed parallel to the ora serrata and helps reduce radial retinal traction. On the other hand, radial relaxing retinectomies can relieve both radial and tangential tractions, reducing the risk of retinal folds at the end of the surgery. The use of endodiathermy is essential to prevent hemorrhages during the retinectomy. The use of perfluorocarbon and fluid–fluid exchange reduces the risk of retinal slippage at the end of the procedure.
