Our guest surgeon does a great job with a very difficult cataract surgery. The patient has phacolytic glaucoma where the lens proteins have liquefied and induced inflammation, glaucoma, and synechiae. Look carefully at the picture, at the tip of the yellow arrow, and ask yourself, "What is going on here?" How would you approach this case? Many obstacles to overcome: small pupil, synechiae, dense cataract, and perhaps compromised zonular support too. And what if this patient is like one of my Beverly Hills patients who still expect surgery to give them the perfect vision of a teenager?
