In this renal physiology lecture, Professor Zach Murphy explains the critical concept of renal autoregulation—the kidney’s ability to maintain a stable glomerular filtration rate (GFR) and renal blood flow (RBF) despite fluctuations in systemic blood pressure. This finely tuned mechanism ensures consistent filtration and homeostasis, even during daily physiologic stress. We begin by outlining the two primary autoregulatory mechanisms: the myogenic response and tubuloglomerular feedback. The myogenic mechanism involves reflexive constriction or dilation of the afferent arteriole in response to changes in blood pressure. At the same time, tubuloglomerular feedback relies on the macula densa of the juxtaglomerular apparatus, sensing sodium chloride concentration and adjusting arteriolar tone accordingly. The lecture also introduces how adenosine, ATP, nitric oxide, and prostaglandins mediate these responses. You’ll learn how these systems act locally—independent of systemic hormones—to maintain GFR within a narrow physiologic range.
