The vastus lateralis can carry a lot of protective tone in knee pain patients because it's a very important force absorber. That means if a muscle synergist isn't doing its job, from my experience, the vastus lateralis usually has to do a lot more work. When the load on the muscle exceeds its capacity, that is usually when knee pain or tendon issues may start to appear. I personally prefer some direct hands on treatment to reduce the tone quickly & then pair it with a split squat for longer lasting changes. A slight variation that can really make all the difference. Here's how to modify the split squat to target the vastus lateralis more: Start with the patient in a split squat position, front foot slightly internally rotated to about 10 o'clock position on the right foot or 2 o'clock for the left foot This small foot position change will alter the line of pull on the patella As they descend, cue them to keep the knee travelling straight ahead The knee should move forward at the same speed as the weight shifts back towards the heel This movement strategy will help the vastus lateralis do work in coordination with its synergists (Soleus etc). For the ascent phase: Cue them to push through the midfoot This gets the soleus helping the vastus lateralis You'll also engage both medial and lateral hamstring fibres To really increase the load through the vastus lateralis, try: Instead of pushing straight up Cue them to slightly lean laterally (towards the vastus lateralis side) as they ascend Think "shoulder coming outside the vastus side" The beauty of this approach is we're not isolating the vastus lateralis in an unnatural way. We're loading it with all its natural partners - the peroneals, soleus & hamstrings - all working together as a team. This distributes the load well amongst the synergists & avoids overloading the knee.
