Internal Limiting Membrane Peeling in Rhegmatogenous Retinal Detachment- Unedited

Peeling internal limiting membrane (ILM) is important step for many vitreoretinal surgical procedures. Purpose of this step is varying in different pathologies. In rhegmatogenous retinal detachment cases, purpose is to ensure separartion of posterior hyaloid from retinal surface as well as avoid PVR changes ie formation of Epiretinal membrane with macular pucker formation at posterior pole. ILM peeling in rhegmatogenous RD is challenging since there is lack of adequate counter traction as in cases of attached retina. Many surgeons prefer to use a bubble of perfluorocarbon liquid (PFCL) over posterior pole. In this technique initiation is easier then when performed without PFCL but progression of peeling is more difficult under the heavy liquid. In cases where it is performed without PFCL, initiation is tricky but further peeling is easier. I prefer latter technique.
