RetinaRounds™ 12: Sub-ILM Hemorrhage

Sub-ILM hemorrhage can occur due to a variety of conditions including Valsalva retinopathy, Terson's syndrome, blood dyscrasias, RAM and trauma. In this episode, Professor Hany Hamza from Cairo, Egypt, demonstrates the surgical evacuation of a sub-ILM hemorrhage in a patient who presented with severe vision loss after severe and repeated vomiting. While patients with sub-ILM hemorrhage can be monitored for spontaneous resolution, more severe cases may require intervention to prevent hemorrhage-associated cytotoxicity to the retina and ERM formation. Therapeutic interventions include Nd:YAG laser to puncture a hole in the ILM, thereby allowing egress of trapped sub-ILM hemorrhage, as well as surgical evacuation. Here, surgical evacuation resulted in an excellent anatomic outcome and improvement in vision from 20/200 to 20/20. Thank you, Professor Hamza, for sharing this very interesting case!
