SQUARE Hospitals Ltd. Obstructive jaundice History of ERCP with plastic Billiary stenting FINDINGS : PAPILLA: Post papillotomy papilla. Blockage plastic stent is seen in situ. The stent is grasped by forceps and extracted out. PANCREATOGRAM: Not attempted. CHOLANGIOGRAM: There is a sharp narrowing is seen at distal to mid CBD. Rest of the CBD, CHD and the intrahepatic ducts are grossly dilated. Trans papillary brush cytology taken. A 6cm covered retrievable metallic stent is implanted into the bile duct across the narrowed segment with establishment of free flow of bile. Huge billiary mud and pus coming out through the stent is observed. Ducts toileting done with saline irrigation. CYTOLOGY: Trans papillary brush cytology taken. COMMENTS: Findings are highly suggestive of distal CBD stricture (But possibilities of distal cholangio-carcinoma could not be ruled out). THERAPEUTIC INTERVENTION: Removal of Plastic stent and metal stenting of the CBD done.
