Elderly male had difficulty in eating food and drinking liquids and was losing weight for few years. He was not able to sleep because of nocturnal cough and Reflux of fluid during sleep. He was treated with acid and cough suppressing medicines without any response. Finally he was diagnosed with a rare but devastating disorder Achlasia Cardia by esophageal manometry and Endoscopy. Achlasia Cardia is a disease of food pipe where valve of food pipe does not open during eating or drinking and and patient may choking sensation and regurgitation that may produce chronic cough. Earlier this condition was being treated with surgery. Dr Kapil Sharma has performed POEM (per oral endoscopic myotomy) on him at Batra Hospital and Medical Research Centre, New Delhi. In this technique tight valve of food pipe is cut and open with endoscopy without any cut on the skin this treatment leads to immediate relief in chest pain , cough and difficulty in eating and patient was discharged after 48 hours. This is a painless procedure performed under general anaesthesia.
