Composition: Composed of magnesium, ammonium, and phosphate. 2. Formation of Struvite Stones Bacterial Enzyme Urease: The key factor in the formation of struvite stones is the presence of bacteria that produce the enzyme urease. This enzyme breaks down urea (a waste product in urine) into ammonia and carbon dioxide, increasing the pH of the urine. Alkaline Urine: The ammonia produced makes the urine alkaline (high pH), which is a conducive environment for struvite crystals to form and aggregate. Crystal Formation: In this alkaline environment, magnesium, ammonium, and phosphate from the urine bind together to form struvite crystals, which can grow rapidly and form large stones. 3. Contributing Factors and Associated Diseases Urinary Tract Infections: Chronic or recurrent infections with urease-producing bacteria such as Proteus mirabilis, Klebsiella, Pseudomonas, and certain Staphylococcus species are the primary contributors. 4. Treatment and Management Antibiotics: To treat the underlying infection. Surgical Removal: Often necessary due to the size and shape of struvite stones. Techniques include extracorporeal shock wave lithotripsy (ESWL), ureteroscopy, and percutaneous nephrolithotomy.
