Are Immediate Implants with Immediate Provisional crowns an indicated solution for highly demanding esthetic cases? Can the Digital workflow and guided surgery simplify the process, or is it adding new dimensions of complexity? How is the planing to be conducted and which are the potential risks for complications? Finally, what does the evidence say for the long term clinical outcomes (and in particularly esthetics!) with this technique? Using an actual case, we attempt to respond to the questions above with a small educational video, aiming to highlight the most important parametres of a complex procedure. Our patient is a 28 year old female, very conscious of her oral health and with high aesthetic demands from her smile. Actually, it is exactly this that brings her to us, as she recently noticed discolouration and slight mobility of 11. After radiographic examination it was obvious that 11 suffered from external resorption which had consumed most of its root, thus the observed mobility. Both the local anatomy but also the patient's wishes, are compatible with an immediate implant and immediate provisional crown. The patient has excellent periodontal health, absence of any kind of inflammation, an intact buccal bone wall of adequate thickness and enough bone volume apically of the socket to ensure primary stability. A recent prospective study by Ivanovski et al followed up implants placed with this technique for an average of 4 years and documented highly predictable tissue stability. In addition, it showed that Pink Esthetic scores remain very high, if not even slightly improve with time. Of course, one has to note that the pink aesthetics were already high when the teeth were in place. This technique aims to maintain aesthetics, not re-create it. The digital workflow, although will not reduce the complexity, can help us increase efficiency and reduce chairside time. Digital or conventional however, careful patient selection, detailed plan of every step and experience remain the essential keys to success.
